i too have experienced this kind of issues and i was using ovpa 4.70

OVPA and OVO work closely togather through CODA.

when i was having OVO 8.60 or OVO 8.53 , i get the above issue frequently.

so i did upgraded from ovpa 4.70 to ovpa 5.x and the issues have vanished.

Note : you can upgrade to OVPA 5.x, if the OVO version is only OVO 8.x

so try upgrading !!
